If you do not already have a CAA website log in, then you will need to create one. The process is outlined in detail in CAA publication CAP1902. 

Please note that this can take up to 10 working days for the CAA to action, so please complete in good time for your medical appointment.

You willl need a form of photographic ID (e.g. passport or driving licence) and a proof of address (e.g. driving licence, council tax or utility bill). The two forms of ID can not be the same document.

Once your application is accepted you can proceed to the next step.
